1. Billing and Payment Terms

Monthly Statements are issued within the last week of each month, detailing all services and
charges for that billing period.

Payments are due by the 15th of the month following the statement date.

Liberty Orthodontic Laboratory does not extend credit. All services are provided with the
understanding that payment will be made in full upon receipt of the monthly statement.
A late charge of 5% of the outstanding balance or $39.99 (whichever is greater) will be applied to
pass due accounts. These charges will accrue monthly until the balance is paid in full.
 

2. Delinquent Accounts
Accounts pass due over 60 days will be suspended from placing new orders.
Accounts pass due over 90 days are sent to collections and incur a 30% collection fee,
Outstanding balance must be paid in full before your account is Reinstated.
 

3. Payment Methods and Fees
A 3% processing fee applies to all payments made by credit card.
Price changes will be communicated by email at least 30 days before implementation.
 

4. Remake Policy
Products are contingent upon the quality of impressions and models provided. Full laboratory fees will apply for remakes caused by inaccurate impressions/models. All remake requests will be evaluated individually. The original appliance and models must be returned for evaluation.

Any disputes regarding laboratory fees must be addressed within 10 days of the statement date.
 

5. Warranty Terms

We guarantee the quality of our appliances and stand behind our work.

• Appliances with manufacturing defects will be remade at no charge if returned within 3 months of the invoice date.

• Fit issues – If returned within 30 days, we will remake at no charge. From 31–90 days, a 50% remake fee applies.

• After 3 months, no returns or remakes will be accepted.

Limitations

• Warranty does not cover remakes due to inaccurate impressions, scans, or incomplete prescriptions.

• Appliances must be returned with the invoice number for warranty review.

• Shipping  and rush fees for remakes are the client’s responsibility.

6. Acceptance of Terms
By engaging services with Liberty Orthodontic Laboratory, you acknowledge and agree to the terms
and conditions outlined above. This agreement is binding and enforceable under applicable laws.
